I agree to return the home sleep testing equipment to Ez Sleep Technologies, LLC in the prepaid return packaging provided, with a postmark of no more than five days after the date I received it. I understand that I am responsible for the testing equipment until the prepaid, pre-labeled return box enters the mail circulation; if it is lost or stolen while in my possession, I am responsible for the full replacement value of $4,995.00. I agree to return the equipment intact and undamaged; I am responsible for the repair or replacement cost of any damaged or destroyed equipment.

I understand that after I provide my payment information for the order, my credit card will be charged for the home sleep test service which includes the use of the testing device, the scoring of my test data by a Registered Polysomnographic Technologist (RPSGT), a test report that Ez Sleep provides, which is interpreted by Sleep Physician and the cost of USPS Priority Mail shipping both ways.

I understand my results report can only be generated after the testing equipment has been returned to Ez Sleep. A $200 hold will also be placed on my credit card in the event that I fail to return the home sleep test within 5 days after it is delivered to me. I understand that at that time if I have not placed the test back into the mail circulation, I will then be charged for the $200 hold that was placed on my credit card.

Seven days after the hold has been charged, I will be charged $4,795 which is the value of the device ($4,995.00) minus the $200 hold already charged. If the device is returned to Ez Sleep after the $200 hold or the replacement fee has been charged, and the device is found to be in good working condition, the $4,995 replacement fee will be refunded minus $25 per day fee since the $200 hold was charged. *Note: The replacement fee will only be refunded for up to seven days after it was charged. After that, Ez Sleep must order replacement units at a cost of $4,995 each, so no refunds will be given after that time.

I understand I need to wear the device all night, however, the device needs to record a minimum of 4-6 hours of total sleep data for the sleep physician to be able to interpret my results.

If I return my device with either no data or insufficient data for interpretation, I understand I will have the option to pay an additional fee for a second test to be sent to me. Once I agree to the EZ Sleep Test Terms and Conditions, I understand no testing or late fees will be refunded even if I cancel my order or return my test unused.

I certify that I will only use the home sleep test on myself.
