I agree to return the home sleep testing equipment to Ez Sleep Technologies, LLC in the prepaid return packaging provided, with a postmark of no more than five days after the date I received it. I understand that I can communicate to Ez Sleep if I need an extension.  I agree to return the equipment intact and undamaged and I am responsible for the repair or replacement cost of any damaged or destroyed equipment.

I understand that after I provide my payment information for the order, my credit card will be charged for the agreed out of pocket amount for the home sleep test service which includes the use of the testing device, the scoring of my test data, interpreted with diagnosis by a Board Certified Sleep Physician, final results report that Ez Sleep provides to the referring Dr, and the cost of USPS Priority Mail shipping both ways. Standard out-of-pocket cost not to exceed $295.

I understand my results report can only be generated after the testing equipment has been returned to Ez Sleep. Ez Sleep retains the right to charge a late fee of $200 if the equipment is not returned within 5 days unless an extension request was granted.

In the event of another 5 days passing without any communication, Ez Sleep retains the right to charge another $200 late fee and subsequest late fees may be added for each 5 day period unless Ez Sleep has approved a delayed return. In the event the equipment is never returned, Ez Sleep retains the right to charge up to $4995 which is the full value of the equipment.

I understand I need to wear the Sleep Testing device all night, and I am aware the device needs to record at least 4-6 hours of sleep data for the sleep doctor to be able to interpret my results.

If I return my device with either no data or insufficient data for interpretation, I understand I will have the option to pay an additional fee for a second test to be sent to me. Once I agree to the EZ Sleep Test Terms and Conditions, I understand that no testing or late fees will be refunded even if I cancel my order or return my test unused.

I certify that I will only use the home sleep test on myself.
